Lifestyle Solutions acknowledges the Federal Court’s decision to impose a $2.5 million penalty in relation to the physical violence and abuse that occurred at a Supported Independent Living (SIL) residence in New South Wales between November 2019 and October 2021.
We are deeply sorry for failing to provide the standard of care that the people we support deserve. We are saddened by the harm caused to the individual and their family, and we offer our sincere apologies.
Lifestyle Solutions takes this matter extremely seriously. Since these events, we have implemented significant organisational reforms to strengthen the safety, quality and oversight of our services. This includes an overhaul of our operating model, enhanced risk management processes, clearer communication and escalation pathways, and strengthened training and support for our workforce. These improvements are designed to ensure robust safeguards are in place across all our services.
To specifically address the conduct underlying the Reportable Incident contraventions, we have installed comprehensive incident reporting and management procedures. These include new measures to ensure rapid, transparent responses to concerns and timely reporting to the NDIS Quality and Safeguards Commission and other relevant bodies.
Our organisation has a long history of working closely and cooperatively with the NDIS Quality and Safeguards Commission, the people we support, and their families, and this will continue. We take our obligations as a registered NDIS provider seriously and remain committed to delivering safe, high-quality services that uphold the rights, dignity and wellbeing of every person in our care.
The actions taken since these events have fundamentally strengthened our safeguarding practices and service quality. The reforms now in place are designed to prevent a recurrence of the failures identified in the proceedings.
