Life Choices & Active Ageing

​​Life Choices and Active Ageing give you the flexibility, choice and voice to select and take part in day activities that fit with your lifestyle and support your individual goals.

By delivering the supports and activities you decide on, we'll assist you to build independence and create strong community connections through meaningful leisure, recreational, social and cultural activities.

Using your individualised funding through Life Choices or Active Ageing you have the freedom to choose:

  • How your support is delivered;
  • Who provides your support; and
  • What activities you take part in.

Am I eligible?

Life Choices is for adults between the ages of 25 and 54 living with disability.

Active Ageing is for adults between the ages of 55 and 64 living with disability, as well as individuals with early onset ageing.

You may be eligible for Life Choices or Active Ageing if you are an adult with moderate to high support needs who is not undertaking full-time paid employment or education.

How do I access Life Choices or Active Ageing?

First, you'll need to contact the NSW Government's Ageing, Disability & Home Care Day & Post School Programs or call them on 1800 761 030.
