Younger people in residential aged care
The purpose of our YPIRAC program is to provide more appropriate living options and practical support for younger people with disability who are currently residing in, or likely to enter a residential aged care facility.
OurYPIRAC program is flexible and focuses on matching an individual's living arrangements with their support needs.
The program allows younger people requiring full time care to create a fresh start away from aged care facilities in an accommodation setting better suited to their exact support needs.
Through consultation with the individual we support and their family and/or carers, we establish a care plan with a range of short, medium and long term goals. These goals focus on:

Through this planning process we:

Develop an effective support network to deliver the care plan and help individuals realise their goals

Identify an array of flexible Day Programs, specialised support services and self managed packages that we can provide to meet the individual's support needs.
By shaping the program around the individuals we support the YPIRAC program not only identifies more appropriate living arrangements, it also links people to appropriate networks and offers specialised support and advocacy throughout the process.
