Lifetime care and support

Through our Lifetime Care and Support Program we provide specialised support services to people who have been severely injured in a motor accident.

Our Lifetime Care and Support program is designed to improve the quality of life for people left with spinal cord injury, moderate to severe brain injury, multiple amputations, severe burns or permanent blindness as a result of a motor accident.

Through a comprehensive assessment and consultation with the individual, their family and/or carer we develop a range of short, medium and long term goals specifically designed to:

• foster independence
• promote self-reliance
• encourage emotional development
• support psychological progress

We are then able to develop a tailored Support Plan which will include:

• physical support and health care plans
• strategies to manage challenging behaviour
• community activities to promote social integration and independence
• strategies that focus on participation, psychological adjustment and reduced carer stress.

Throughout this process we:

• collaborate with Social Services to provide continued support for the individual and their family within the home setting.
• provide links to peak bodies for the provision of specialised support, information and activities.
• collaborate with families and carers to ensure rehabilitation programs are integrated into the individual's daily activities.
