leaving care program
Moving away from out of home care represents a massive milestone in life that brings with it uncertainty and change. Our leaving care programs are designed for young people making this transition to ensure this journey is a planned, positive and rewarding experience.
Drop In support
Participants in the Drop In Support Program are allocated a Support Worker who provides drop in support for a designated number of hours each week. Together they will develop a care plan that focuses on:

Setting and achieving personal goals

Developing skills
-
Connecting with the local community

Establishing formal and informal support networks
During their visits, the support worker will offer assistance across a wide range of activities and lifestyle choices.
The comfort and confidence that drop in support brings has a really positive impact on the lives of the people we support. It is also very reassuring for our participants to know that they have access to 24 hour on-call support 365 days a year.
Alternate Family Placement
Through Alternate Family Placement, participants enjoy the comfort of a caring, safe and supportive home environment with an approved Family Carer.
Each individual is also allocated a Lifestyle Solutions Planner and together they will develop a care plan that focuses on setting and achieving personal goals.
Weekly contact with our Planner will assist the person we support and Family Carer in a range of areas including:

Developing independent living skills

Making decisions and negotiating change

Accessing educational and vocational opportunities

Pursuing community, cultural or religious activities

Developing and maintaining positive relationships

Maintaining good health and emotional well being

Reviewing accommodation needs
Our participant and Family Carers have access to 24 hour on-call support 365 days of the year.
