Disability services

Our Disability Support Programs provide person centred support to people living with disability.

The support offered through our Disability Services can be provided to individuals:

• in the family home
• in the community
• through centre based activities

As a preferred supplier of YPIRAC funding, we are dedicated to working with and respecting the families of the individuals we support.

Lifestyle Solutions supports and encourages family relationships, respecting the natural authority of each person's family and their cultural background.

Our Life Choices Programs focus on the skills required to support individuals to confidently transition to independent living. At the same time, we acknowledge the adjustment process for the family s a whole.

These Community Day Programs vary to match the skills and interests of the people we support and always focus on:

• Personal development
• Education
• Recreation
• Independence and social participation

Importantly these day programs also provide respite for the family.

There are three support models available:

• Centre Based Support with Community Access
• Individual Community Based Options
• Self-managed Models of Support
